Salary Range: $58,000-$65,000 (Pay information will vary depending upon relevant experience for the position)
Full Time 40 hours

GENERAL SCOPE: General Responsibilities include case management and therapeutic intervention in the psychosocial environment of patients on short-term rehab to promote the patients optimal physical, psychological and social functioning throughout their rehabilitation.

EDUCATION / QUALIFICATION: Bachelor’s Degree in Social Work required. Master’s Degree preferred.

EXPERIENCE: Prefer one (1) year experience in a long-term care setting or related setting.
